New User Request Form

I would like a new account on (check all that apply):
Homestead (or 'rcf', SGI Origin 300)
Prairiefire
Merritt (new Altix machine)

Please choose one of the following. If you are a new user, you can request a specific username by filling out 'Username' field. If it is blank, we will generate a username for you. If you are an existing user, enter your username in the 'Username' field.
